First Advantage Background Screening- PLEASE ADVISE [CO]

Howdy! I wanted to know if First Advantage flags unemployment gaps if I listed them under “unemployed”. Also, in the employment verification section, it asked only for dates of employment, title, and company name- no box asking permission to contact employer. How will they verify employment if I gave them no contact information? Why wasn’t I given the option to select yes or no to FA contacting my employer like many others have?

Also- do they run credit checks? The job I was offered is within the healthcare sales industry and am wondering if they can rescind an offer based off of poor credit (mid 400s/lots of credit card debt. I know it’s shit but had to heavily rely on credit cards since I had no job to pay my bills) prior to then my credit was in high 700s.
