Help getting gender affirming care

Hi, my first post about transitioning in some manner. I'm 23 (NB) on my own health insurance through work (unfortunately my bio donors still have me on theirs. But it shouldnt be my main. I'm no contact with them).

I am AFAB, and want to get top surgery & sterilization. I also would like to go on a low dose of T (applied downstairs) with no masculinizing effects.

I've never been out about my experiences except as of late, and I'm not sure where to start. My friend got their top surgery at a major hospital with support from their parents. They also went to planned parenthood for a low dose of T.

I'm just concerned that when I go in for assistance, I'll be told I look too much like a woman. I've tried talking to my PCP about this, and she is helping me with vaginismus (sending me to a pelvic floor therapist).

I just want to be taken seriously.
