Updated to add important info and more test results: What is this rash?

Edited to add some important info. I know Reddit cannot diagnose me. I have gone to urgent care, and have an appointment with my PCP provider to, hopefully, get a referral to a rheumatologist. But, I can't be seen by my PCP until August, so I am left to sit, think, and worry.

I was prescribed Triamcinolone by the urgent care doctor and recommended to see a rheumatologist ASAP. The Triamcinolone is doing nothing for the itch.

I guess I'm just here looking for similar experiences prior to diagnoses of lupus, rheumatoid arthritis, lymphoma, etc. And some opinions on what is going on with my body? I was told at urgent care that it's possible it could be one of these three things, but to start with a rheumatologist to address the skin problems?

27, 5'3, 144lbs, Female, no meds, daily smoker of cigs and weed for ~12 years.

I developed this horribly itchy rash about two months ago. It's only on the back of my neck. It burns occasionally. Haven't switched any soaps/products/detergents/etc. No lotions/cortisone creams/allergy pills are helping. No history of eczema or other skin problems, other than PMLE that developed within the past few years.

The lymph nodes in the back of my neck are swollen, as well as the ones in the sides of my neck and behind my ears. I have a history of rock hard swollen lymph nodes in the sides of my neck and back of my ears, the first one noticed in the left side of my neck maybe 5 years ago? The back of the neck lymph nodes swelling came after the rash.

It's incredibly itchy, burns occasionally, and radiates heat/hot to the touch. I now have some open sores that won't heal where the rash is.

My lymph nodes also seem to itch. Or rather, the skin on top of the lymph nodes, even where there is no rash.

Before I let Google convince me a simple case of eczema is lymphoma or some other kind of skin cancer, I thought I'd ask Reddit.

The neck rash

Edit 1: I forgot to mention in the original post, a couple months before the neck rash appeared, I started getting these circular, occasionally itchy patches on only my legs and stomach. I also have one on my foot.

These are the same spots that originally popped up. They don't go away or "heal." And they occasionally itch, but nothing like the neck rash. They actually aren't even really noticeable and don't bother me until I take a shower or something. It seems the heat and/or water irritates both the neck rash and the patches on my legs.

Edit 2: I have several of these spots on my legs, 3 on my stomach, and 1 is on my foot in the first set of photos

I also have a lump in my lower left leg, on the internal side of my shin bone. I have some bumps from bruises over the years like most people, but this is definitely a lump of something. It feels just like a hard, round, swollen lymph node and it's about 1cm in diameter. The skin is slightly pink over it, but the lump is internal. Are there nodes in the lower shin area?

Edit 3: Added image of most recent CBC (October 2024) if this is in any way helpful. It was marked as "abnormal"

Edit 4: Basic Metabolic Panel (Oct. 24) abnormal and elevated D-dimer from Oct. 24

and a Visibly swollen lymph node in my neck currently

I've been dealing with swollen lymph nodes, malaise, fatigue, and a boatload of other problems for the past couple of years, plus losing about 100 pounds over the past 2 years without trying at all. I'm convinced I have some type of slow-growing lymphoma.

My tonsils are also different sizes and weird-looking

I have no appetite whatsoever. If I do feel hungry, I have about a 5-minute window that I'm able to actually eat (and I use it wisely and shovel as much food down as a I can) until my appetite is gone again. I can only eat once a day, if I'm lucky to have the appetite that day.

I feel WIPED. Things that shouldn't make me feel entirely spent, do. A 5-minute walk with my dog turns into me having to stop once or twice to sit in the grass, or on the sidewalk. I can't explain the relief I feel when my body hits my bed. I know what being tired after a long, busy day feels like. This is exhaustion that doesn't go away after rest. It's there after mere minutes of physical activity, even if it's just light cleaning or something. I feel like I am going to collapse. And like my limbs and everything are just so, so heavy.

I also have polymorphic light eruption that appears at the beginning of every summer for the past 3-4 years. Only on my arms.

HIV negative.

Negative for Lyme Disease, ANA normal (less than 1.8), rheumatoid factor <10, thyroid 1.20 mcIU/mL, sedimentation rate 13, comprehensive metabolic panel with trace protein in urine (occasionally foamy pee)

I did have mono as a teen, about 11-12 years ago, with very very high levels of the Epstein-Barr Virus, according to the pediatric medical records I requested.

Another photo of the neck rash with two visibly swollen lymph nodes. One is behind my ear, and I circled the visible node on my neck.

Is there a specific kind of test I should request at this point? Or go straight to asking them to just biopsy one of my lymph nodes, the rash, or something? Should I go to the ER, or wait until August for my PCP?
