Consent Checklist

Me and my girlfriend have gotten pretty into hypnosis but we both get excited for it and can forget to do the important steps before we get started.
So I suggested a consent checklist we have to go through.
What I have so far is

1. <My name> is prepared to be hypnotized.
2. <Her name> is prepared to hypnotize.
3. Is <my name> willing to go past her regular limit (no being made into a sexualized animal)? How far and in what way?
4. How long will this session last?
5. Are there any special limits to put on this session?

What else might we need on our consent checklist?
