Tips for Dark Souls 3 100% achievement

Hey guys! I started playing Dark Souls 3 and I love the game and I'm willing to go for the 100%. I know that I have to get to NG++ for the 100%, but is there anything else I should know before doing it? Like any missables or quests that I should be aware of? What I want to do is the finish the story blind and then doing the clean up after the final boss. Can I do most of the achievements after the story?
