Maternity Leave

To those who went on maternity leave early for no medical reason what did your doctor put on your papers for going early? I’m due early December but want to go on leave mid November but my HR pretty much told me I couldn’t without my Drs permission. I don’t plan on returning to this job so I wont loose that time with my baby.
