Ileostomy prolapse and recurring excoriation

Loop Ileostomy

Looking for tips on managing a prolapsed loop ileostomy and avoiding output getting under the edge of the bag/barrier ring. My ileostomy, Betty Poop, is slightly prolapsed to a max of just over 2" since about a month ago, though it can retract to just about 0.5". However, since Betty really came out of her shell, I'm finding that I'm getting a lot more output finding its way under the edge of the bag/barrier ring. Not to the point where it's a full-blow leak or blow out, but it's always in the same spot, right under the stoma output hole, and it gets painful very quickly. Prior to this prolapse I was able to wear some styles of pants and leggings as long as I wore a stoma guard under it. I have the Halo, which worked super well. However, since the prolapse, the stoma guard actually makes things worse as it presses down on the stoma, which pushes the output hole right on the edge of the bag. Any clothes that aren't really loose do the same thing. All of it results in this one little patch of skin being slowly eaten away, and goddammit but it's painful. I use Coloplast Sensura Mio one piece cut to size bags, Hollister Adapt barrier rings, crusting powder & 3M Cavilon skin prep swabs, and have just been given some hydrocolloidal sheets to apply over sensitive or excoriated patches. I tried a bunch of different bags, and the Sensura Mio works best for me, but all of the bags tend to vacuum seal themselves toy stoma after an hour or 2 (no idea why) which causes excoriation. Here's what I'm doing to reduce pressure on/squashing of the stoma:

  1. Loose clothes
  2. No more stoma guard 😢
  3. Careful w diet and water intake to avoid really thick output and pancaking (which I've found also causes that seeping under the bag edge)
  4. Rinsing bag at each emptying to make sure any thick output is loosened and flushed out, so it doesn't accumulate.
  5. Always trapping a little air in the bag so it doesn't vacuum seal itself to my stoma.

Anyone got any experience dealing with this?
