Supply Dropping at 2 Weeks PP

Newborn Troubleshooting

I am freshly two weeks postpartum and my supply keeps dropping, and I don’t know why. At the beginning of the breastfeeding journey, my breasts were full and leaking, and now they are flaccid and barely able to keep up with my baby’s demand. I put him on the boob whenever he wants, we have more than 6 wet diapers a day, and he was back to his birth weight just 9 days after he was born. What am I doing wrong?

He wants to clusterfeed, and I let him, but there is nothing left in my boobs and he keeps getting frustrated. One thing though, I use nipple shields for feeding him. My nipples are extremely flat and short, he cannot latch for the life of him.

I’ve seen two LC already about this, and as soon as they hear the word ‘nipple shield’, they blame my supply dropping on that. But he pees very often, and is steadily gaining weight? I can’t wean him off the nipple shields, they are my only hope with continuing breastfeeding, which I really want to do.

Everyone tells me to keep him on the boob whenever he wants, and that’s exactly what I do. But what do I do if there is nothing left in them and he fusses because of that? Maybe it’s poor milk transfer? But wouldn’t that affect our weight gain and diapers? Maybe because my let down is too slow?

During the afternoon, my supply seems to be enough though? He feeds on one boob only, transfers lots of milk (I can audibly hear him swallow) and falls happily asleep. I pump after feeding him to try and increase my supply, and the boob I fed him from gets anywhere from 5 to 20 ml, and the other boob will typically get me 40 to 60 ml of breastmilk.

But in the evening and nighttime, my supply drops significantly. Just now, I gave him my right boob, left boob and some expressed milk just to get him to settle. There was very little milk coming out of the right boob, a little bit more from the left and he had 15 ml from the bottle.

Does anyone have any clue what’s going on? The two LCs were not even remotely helpful, and couldn’t even help with the latching issue. Just wondering if anyone has some experience or advice to share.
