Applying to vary undertaking

I was sent forms to fill out to vary my undertaking condition about no contact (because the other person involved wants to open up communication to discuss our jointly owned organization/business). I am going to receive help with my case through legal aid and was assigned a lawyer last week. I called to request assistance but they know there is no legal time limit for this document and my request will likely fall to the wayside. (Even the legal aid office is too busy to take my calls today.)

I would like to fill out and file it myself to save time, however, I don't know legal jargon enough to do it properly. Are there any resources that I could get examples of how to structure my sentences when filling out the two sections: "Consent of the Crown is requested to vary the Release Order or Undertaking as follows" and "the reason on which the application is based are as follows".

(I also would like to change the condition of my ability to go to to apartment so that I may move out the rest of my belongings. I am hoping to file another application if the first one is approved.)
