Extreme fatigue 5 mg - need help

Side Effects

So a little background first:

I have PCOS which caused immense weight gain. And I'm not really a big eater, I typically only ate once a day before my weight gain. I was finally able to start losing weight with the help of a nutritionist and more regular eating.

I am also married and my husband and I want to have a baby, but I am only going to start trying once I am at a safe and healthy weight.

Before zep, I had lost around 40 pounds from November 2024 to May 2025. I had been dealing with other health concerns, most related to PCOS, and going to tons of doctors. One thing that came up is that despite my A1C and glucose being in a healthy range, my insulin was still high.

My dietitian had said that as I continue to lose weight my insulin resistance should improve, however I am nervous about this being unmanaged. I tried metformin and it made me really sick so my next option was a GLP-1. Because my insurance doesn't cover mounjaro unless you are actually diabetic, I figured I'd use Zepbound because I qualify with my weight and sleep apnea.

And to be honest, while this decision was solely made on the basis of insulin management, of course there's a little part of me that likes the idea of accelerated weight loss so I can try to get pregnant even sooner.

2.5 mg was okay. Some stomach pain the day after. Harder to eat much which is an issue since I struggle to eat frequently to begin with. However I'd been able to find other ways to get nutrients and protein. After a month my doctor said my insurance required me to go up to 5 mg.

5 mg has been an absolute nightmare. I feel like a zombie all of the time. Even looking at food makes my stomach turn. When I do eat, I feel like Kristen Wiig in that food poisoning scene of Bridesmaids when she's trying to prove she's not sick by eating the piece of candy. Same agony.

And while I am suffering, I do not want to give up just yet. I don't have another option for my insulin and there's just that nagging part of me that says "if you get through this you can get pregnant". Any sort of advice or tips would be beyond appreciated. But even if you just took the time to read all of this thank you so so so much.
