Headphone/Speaker Icon

This just started tonight. Roku Ultra (4800, I think) is showing an icon in the upper right corner of the tv screen where the volume icon is when you change the volume. Now that icon looks like headphones in front of a speaker box (rectangular old school speaker). No luck with Google searching the icon. The volume number changes but no sound can be heard. I tried headphone mode in all different places but that icon is just headphones. Restarted and unplugged power and plugged back in. Still nothing changes. It seems as though the sound comes out normally a few moments after restart/power cycle but then it goes back to the headphones/speaker icon as though it is connected to something. I can’t find in settings that it’s connected to anything else besides the enhanced remote. All devices were checked for BT connection and headphone jack on remote was cleaned. I may not be looking in the correct settings though. Apps and Ultra are up to date.

Any help would be greatly appreciated.
