25M penis rope like structure all the way around shaft not going away

Just happened right now. I was just in the emergency room for my testicle two days ago and had my penis checked as well. They said it was fine but now the rope like structure on the side that was only on about a quarter of the circumference on the right is now going fully around. I'm on my way and need advice quickly. It is painful, about 7/10
