Cohabitation Evidence for Family Visa (GP letter)

I am currently living in the UK under my first 2.5 year family visa (Married to UK citizen).

For cohabitation evidence we plan to use: Bank Statements, Utility Bills, and letter(s) from the GP.

Me and my partner registered a while ago at a GP under our current address, and I'm planning to visit the GP to request a letter confirming me and my partners address that is registered at this GP practice.

What should I request specifically? (Official name for such a document)

Any examples or advice is appreciated!
