[Arcade] [unknown] Picture Revealing Arcade Game

There was this game I’d play when I was really little, maybe about 5 or 6. You played as a pair of eyes that had to create squares to reveal parts of a picture, and once you’ve revealed enough of it you’d win the stage. There’d sometimes be a weird, filtered voice that said “Look out!” occasionally. I also believe it had a normal images mode and an “18+ images” mode. I wasn’t allowed to touch the latter for obvious reasons. Does anyone recognize this game?
