Me (29F) and my "situationship" (35M)

I (29F) was in a situationship for nearly two years with a guy (35M) I truly cared about. He was always very clear: “I’m not looking for anything serious.” I accepted that - against my better judgment - because I had deep feelings for him.

A couple of weeks ago, after a night of intense sex, I started feeling severe pain. Turns out, an ovarian cyst ruptured — something rare unless there’s extreme physical pressure. I was in agony. While still with him, I told him I needed to go to the hospital. He said he could help call me an Uber but didn’t offer to go with me because he doesn’t like hospitals.

Later that night, I collapsed and had to be rushed to the ER. I underwent emergency surgery for internal bleeding. It was serious and painful.

He visited me in the hospital the first two days, brought a few toiletries. But after that, he ghosted me. He went sightseeing with his friends (he’s a digital nomad) and even posted himself drinking on Instagram while I was still hospitalized — all while saying he “cared.”

When I later expressed my hurt, he said, “We're not a couple, that’s just a fact,” and that he didn’t visit again because I didn’t ask him to — as if the burden was on me.

I know he didn’t physically cause the rupture on purpose. But given the context — that I was hurt after sex, clearly in pain, and going through a surgery — I expected more presence, support, anything.

Now I’m questioning everything. Was I wrong for expecting more from someone who "set the terms" from the start? Or was his behavior lacking basic empathy, regardless of the label?

TL;DR: After a night of intense sex, I had a medical emergency. The guy I was in a situationship with helped minimally and then backed off. Curious what others think — did I expect too much?
