LOR without a letterhead but with a signature

I have a physician whom I am trying to get an LOR from, but he doesn't really have a logo or a letterhead. He already submitted once without a signature and a letterhead, and I've asked him to resubmit already with both but he provided an LOR with only a signature then went on vacation 😭 Is it possible to just submit it as is? He runs a private practice and he's the only doc, so I'm assuming there may not be an official logo or a letterhead he could use, but id be kinda embarrassed to ask him to resubmit twice now. What should I do? For context, I already have a committee letter and some professors, so I'm good on everything except an MD LOR.
