Experience with travel insurance, claims etc.?

I want to travel abroad, and will need to book flight, hotels in advance. But, later I could have a situation like sickness, bereavement etc before the travel date. Does anybody have experience with claims? What kind of proof do they need for sickness, bereavement etc? Do they honour the terms, or do they deny the claims for petty reasons/is the insurance for namesake?
