Is it appropriate to give a doctor a gift before leaving the practice?

I am moving soon outwith of my area, and to a new place where I will need to register with a different practice. The doctor I have has been absolutely wonderful, and I have seen her consistently for the last 10 or so years about various different things, I have only good things to say about her. She really made a difference in my life.

I wondered if it would be appropriate to get her a gift before I leave? I know there is generally some guidance around receiving gifts as a doctor but I’m not sure what it is.

I was thinking potentially a thank you card and some chocolates/biscuits for the practice? I’m unsure if she’d be able to accept the biscuits/chocolate etc for herself.
