Alpha lipoic acid for peripheral neuropathy - has anyone tried?
BACKGROUND: several years before my MM diagnosis in Nov 2024, I had some testing done on my feet due to discomfort (cold feeling) and potential circulation issues (red toes). (I can't remember what the testing was called but they put little sensor things on my feet, ankles and toes and I was there for about half an hour).
My family doctor told me the results showed that I had minor peripheral neuropathy and we would keep an eye on things. It slowly a bit more noticeable over the next year or so but has really started to bug me since my MM treatment began in January 2025.
MM TREATMENT TO DATE: Note: I live in Canada. 🇨🇦. Started with 3 cycles of dex, bortezamib, lenalidomide, zoledronic acid (Jan-Mar 2025), followed by SCT in May 2025, and then 100 days of no treatment except pain management. I feel like the neuropathy symptoms are becoming more extreme as time goes on. Symptoms didn't change while I was on a treatment "holiday" post-SCT.
SYMPTOMS OF PN: It is a painful burning/freezing/tingling feeling. It's so bad it keeps me from sleeping. I experience it 24/7.
My feet don't feel cold to the touch. Massage doesn't help. Magnesium spray doesn't help. CBD lotion doesn't help.
Compression (tight socks) helps.
This month, I begin treatment again - 3 more cycles of dex (lower dose - 20 mg instead of 40), lenalidomide (25 mg), bortezamib (lower dose than during induction), and zoledronic acid monthly.
Oncologist says unfortunately lenalidomide is always going to be part of my treatment and maintenance.
PRESCRIPTION: Oncologist has just prescribed gabapentin but I'm concerned about the sedative effect combined with my hydromorphone (I take the minimal dose) and evening sleeping pill (low dose seroquel). I've also read that ongoing use of gaba can cause cognitive issues and I already have brain fog plus a family history of Alzheimer's.
QUESTION: So, I'm curious if anyone here has used alpha-lipoic acid instead of gabapentin for neuropathy and what their results have been? I have joined the neuropathy subreddit but I'm of course particularly interested in MM patients' experience.
GOAL: I would love to hopefully keep it from getting worse, as I recognize it likely isn't reversible especially since I had less severe symptoms that started approx 3-4 years ago.
Thank you, as always, for sharing your stories and opinions.
