Honour mode is FAR more annoying than it is hard imo.
The amount of equipment, perma bufs, and questlines that you can miss (especially in act 1) is (in OM) astonishing.
You can easily beat honour mode but you can lose so much that you planned to have and it creates holes in your characters that stay. It doesn't make honour mode that much harder if you accidently kill volo, but it removes a character that you wanted to have.
You can lose honour mode in pretty BS ways without the enemies needing to move a finger; clicking a rune powder barrel on accident or a bridge support and not being able to turn back for example. You didn't make the wrong decisions really, you just clicked somthing once that you didn't mean to.
Straight up fights should be the hardest thing about honour mode (I think) yet they end up being mundane activities that you prepare for.
You should be able to build your character freely for the fight to come instead of losing important gear and the "fights" are not even hard anyways, even if you lose a few gear pieces on your journey.
My latest mistake/oversight that motivated me to make this post:
I fought the hag as a tank death knight build oathbreaker/necromancer with tanky gear, and I had karlack the barbarian with the wild heart sub-class. Karlack had the shattered flail to make her tough and she goes crazy when she doesn't do damage to enemies for a turn. I thought I was lucky when I got the upper hand in the bargain, (I rolled a nat 20 and "saved" marinia), but in an ironic and funny twist of fate karlack went mad after the deal was done and it was still turn based combat. Marina punched karlack for 1 damage then karlack retaliated brutally, craking marina's skull in one hit. Now I can't get conner to complete my death knight build because of a really specific interaction.
