Question about employer insurance

Every job I’ve had I would have to fill out a form of the insurance provider if I was accepting or denying it. My current job that I’ve been at for 7 years I’ve never had to do that, even when I had the insurance I never got any paper work. Im concerned they are filling these forms out without my knowledge. Is it mandatory to accept or decline insurance every year?
