Do I have any chance?

I finished undergrad about a year ago with the absolutely abysmal GPA of 2.4. However, realizing shortly before the end of my undergrad that I wanted to be a PT, I enrolled in 2 community colleges and have been taking prerequisites and other classes to boost my GPA. Now my cumulative across the 3 schools is around a 3.1, with my science/prerequisite GPA being around a 3.8. Aside from just grades, I work as a PT aide, and have gained shadowing hours in various settings. Will PT schools be put off by my horrible academic past despite my improvements? I plan to apply to PT schools next summer, but if it is impossible at this point I might just try another route. Please let me know what you guys think.

I should also note than most of my prerequisites were done at these community colleges.
