I wish everyone would label their deviation from common standards.

It should be in bright colors with a label hanging over each connector.

It looks like the holes in the rack mount bracket on the left are not aligned properly... and a screw is missing... That and the APC/UPC problem.

It looks to me like they just had an APC patch panel, but had a need for some UPC connectors as well.
Instead of getting blue LC couplers to replace the green ones in the patch panel, they just labeled the deviation.
The connectors on the other side on those ports are probably Blue/UPC, and a quick pop of the patch panel cover would confirm this.

Am I missing something or is the subtle fail, that you split the slots on the rack?
Those green connectors - that signifies APC (angled physical contact) singlemode fiber. The proper color for UPC (ultra physical contact, no angled connector) is blue. If you just went by the connector color, you'd be connecting an APC patch cable to a UPC fiber, and you'd now be wondering why your link is either a) not up at all or b) suffering significant attenuation. Even worse, if you have high power optics, like say in a DWDM system, light reflecting from the mismatched connector can break the optics.

But, which one is wrong? the connectors or the label?
That was all I could see, so I think that's it.
Green connectors are APC. Angled polish connectors.
Why even label them? Plugging an APC into a UPC (or the reverse) will damage both connectors (and if you damage that box, that's going to be damn annoying).
Now that I think about it though, its also possible that the LABEL is wrong and they really are UPC and the "real" problem is that the LC connectors are green instead of the standard Blue which IMO is an even bigger screw up. Either way, I'd be entirely unsure of what to plug in there. Optical microscopes ftw!
