PCP ID number in New Hampshire

Greetings all, I don't know if this is the correct subreddit to ask this but here we go. I'm trying to add my wife to my health insurance that I get through my employer. They require her pcp's id number. I've scoured Google and I get directed to an NDI number? The only info we have is her pcp's first and last name. Any suggestions/guidance would be extremely helpful!
