Diabetic cat

Newly Diagnosed

I need some advice…

Annie is a 3-year-old Bengal cat who was diagnosed with diabetes a month ago. It’s been a nightmare—she was hospitalized twice and went through ketoacidosis. I honestly thought I might lose her. Her fructosamine was 733 and her glucose level was 24.

The vet started her on Lantus, 2 units in the morning. In the hospital, her blood sugar stayed nicely around 10–12 mmol/L for 24 hours.

But once she came home, she started eating normally again, and her glucose began to rise (she barely ate in the hospital and was on IV fluids). At home, her levels went back up to 16–21 mmol/L. The vet then recommended increasing her dose to 2 units in the morning and 2 in the evening. This didn’t make much difference—her sugar stayed around 15–20 mmol/L.

We then tried adjusting the dose to 3 units in the morning and 1 in the evening. This worked for just one day—her glucose stayed around 10–11 mmol/L during the day, dropping to 9 mmol/L by the evening. But the effect only lasted a day, and then her sugar shot up again. Today, before her evening insulin, I measured her glucose at 27.7 mmol/L!!!!!

We’re checking her blood sugar regularly, every 2–3 hours when changing doses, and she has only once reached 7 mmol/L, never lower. I’m completely at a loss—what should I do? She is, of course, on a low-carb diet.
