Transferring ownership of vehicle in NY, but I live out of state

My mom bought a car in NY a few years ago under her name which I now physically own and use. It's been paid off and now I want to transfer ownership of the vehicle to myself. The certificate of title is a NYS one and so are the license plates. However, I live in Wisconsin now (with the vehicle) and I need to get Wisconsin plates and a title under my name

How can I get the vehicle transferred to my name?
