Swelling around penis

I have a redness and swelling on the lower part of my penis from a long time like 6 months or maybe a year,and there’s some redness and uneven thing around the urenary passage also,when i touch it after removing the foreskin it pains,also when i remove the foreskin and try to have sex,it pains and i cant get intimate confortably,what should i do I’m very scared
