Is head on bugged?

This is the 2nd time I've noticed this. Does head on not activate if you enter a locker while exhausted but regain exhaustion while inside and then go for the stun? Because when I try to go for the stun after recovering exhaustion while inside, I get the prompt that the perk is active, the notif for stun points, and the exhausted status again but with no stun effect given.
