PA: Power of Attorney or something else for poly relationship?

Location: Philadelphia PA, I'm poly and married and I have some long terms partners. My question is this: my wife has power of attorney in the case of anything happening to me, but I also want my partners to have some say potentially too and if nothing else, I do not want them ever barred from seeing me in the event of a medial emergency. My wife knows all this and there's not even a question that she'd be consulting with them and giving them access to me. My concern is more around hospitals being shitty or just weird situations where both my wife and I are in the hospital or whatever, I have no clue, I just want to make sure they're treated as family to me even if I'm not legally married. Is a structure like this allowed? Maybe I'm not looking for PoA but something else instead?
