Notice Of Action Ontario Civil

:24px-Flag_of_Ontario:Ontario

As the title suggests I am looking for some information about filing a notice of action in Ontario.

It will be in Superior Court as it is above the threshold for Small Claims.

My limitation period for filing a Statement of Claim is unfortunately on the 6th and I need more time.

I am wondering exactly what information needs to go in the Notice of Action.

Examples I see obviously include the claim, but I don’t have all that information available to me(due to the criminal trial being ongoing and my access to information being impeded).

I absolutely need the 30 days extra(and even then I am limited as the criminal trial isn’t until November), but I am concerned I will not have all the information I need on the Notice of Action.

I understand I will need the party names/addresses/etc.. but am at a bit of a loss as to the rest.

I very much so appreciate any and all advice one may have as this is a very stressful situation and I am deeply concerned about errors.
