Laminectomy denied

Have 3 herniated disc stenosis and an annular tear. Two qme's have stated I needed surgery to improve. Neuro surgeon has put in for laminectomy twice and has been denied twice. Not sure what the next step will be. I have not worked in over 2 years and recently my wc ttd has been cut down to $290 a week but I've been told by my attorney that can stop at any time. I have had 2 epidurals and physical therapy with no improvement. Any ideas of how I should proceed I feel stuck. Thanks for your time
