Did I Get Scammed? Parking lot windshield repair

It didn't occur to me until after the gentleman left, I am horrible in situations like this, which is why I usually try not to shop during peak hours. The guy took a bunch of information, gave me a pretty legit looking business card/flyer, and he took my insurance info down. Wasn't until he walked off I remembered that not everyone is just trying to do legitimate work and I might have just given away a ton of stuff I shouldn't have.

So what's the likelihood I just got my identity stolen? Is there anything I should do? Call my insurance and tell them to disregard any claims made by third parties? Accept that my life is ruined? Or am I overreacting and things will be fine?
