Applying for access arrangements

I'm applying for access arrangements but I'm slightly worried as my full assessment has details about my drug history saying I've taken recreational drugs and listing them, saying the last time was a year ago.

I'm pretty worried to submit this report to the icaew, has anyone had a similar experience? Maybe I'm just being paranoid as it's an irrelevant part of my medical history. I've contacted Psychiatry UK to ask them to remove the couple of sentences about it and they won't.

And my diagnosis letter won't be sufficient, what shall I do?
