Anyone else who is a completionist getting frustrated with undiscovered locations on the map staying 'undiscovered'?

Still got to say I still cant believe this subreddit doesn't allow Image posting xD. Anyway, screenshot here for an example.

I been having this frustration when going to an undiscovered area its still undiscovered. I cant hug the "?" icon any more than I already have. Its a hit or miss for some areas. Some areas it just registeres and is 'discovered' fine. Times like this its still showing I never been there. Not sure if there's a certain quest you need to trigger, or a certain enemy encounter or what have you. If anyone has any idea, please let me know I'd appreciate it.
