Best way to get it to.. poke out

So I've been on t for 14 months now and I have a decently sized tdick I think but I can't get it to like poke out from my labia they just kinda hold it inside and when I look down all I can see is puss. I know pumping makes it bigger for a while but I want it like always ya know but I'm so scared it's not in the cards for me
