Birth certificate in 1986

Does anyone know what the look and format of birth certificates were in North Carolina in 1986?

I need to write out a short part of a birth document listing the child's info and parents' names, but I wanna be accurate. I've tried looking it up, but have fallen short. Unsure if I should just go with a generic format, but if y'all have any in mind that might work well, please let me know.
