Lvl scaling in hard

Hello everyone,

Does the lvl scaling have some issue? Or the main quest and some open world thing scale very high ln hard? Or don't scale at all?

I sometime have some enemy i didn't see their lvl. I wanted to finish the main quest before, see that and decide to do some side quest, but i had the same thing in a quest in the open world (in a place where i was 15 lvl before, so not suppose to be hlvl...). So i'm not sure how it work...
