Covid fears before surgery date

So I'm due surgery in November and covid is making it's rounds again. I'm having surgery in Turkey (flying from the UK) and on the sheet I signed there is a section that states "I claim I didn't get Covid in the last 6 months and I didn't contact anyone with Covid in the last 6 months" how important is the 6 months? I assume it's to do with your health before surgery?
