False charge on my billing

My new rheumatologist charged my insurance $690 for a 60 minute new patient visit. OK, except this guy spent maybe 10 minutes with me. (Maybe 15? It was not long at all.)

Is this insurance fraud? Or is it fairly standard practice to charge what time you allocated to a case instead of what time they allocated for a patient, not what they actually did with a patient?
