URGENT / DESPERATE FOR YOUR HELP : NET, Rapidly Failing Liver, Undrainable Ascites.

Physician Responded

My brother is in a desperate situation. His doctors have been highly transparent, admitting confusion and stating they are running out of traditional options. I am reaching out to anyone who have faced similar "impossible" situations.

The Diagnosis & Status (The Bad News):

  • earlyDiagnosis: my brother was so hospitalized because of liver abscess. He was given antibiotics but unfortunately nothing worked. He then had a colon and fiber scopy. Both didn't show anything. Later, the doctors did a biopsy which showed that there was some tumors on the liver. The initial thoughts was that it was a metastatic liver cancer and they were looking for any primitive. They could not find anything and they ruled out a lot of options. Doctors were really confused so they ordered PET scan which only showed that the cancer is located in the liver (they still at this point do not know where the primitive is. The current hypothesis is that it is located in a small membrane or a vein near the liver.)

  • Current Confirmed Diagnosis: Likely a liver NET because of later biopsy result. Before, doctors thought it was a Hepatocellular Carcinoma / HCC (still didn't completely rule out this option. More info below)

  • Liver Function: Rapidly degrading, with dangerously high liver enzymes. His liver is too compromised for standard chemotherapy, doctors said.

  • Jaundice: Severe yellowness, especially in the eyes.

  • Fever and infection: Resolved only after high-dose Corticoids (steroids). Infection was attributed to a syndrome of HCC, since it was not reduced by any traditional means.

  • Ascites (Severe Bloating/Fluid): Fluid buildup in the abdomen is severe. Doctors are unable to drain due to it being close to liver.

  • Current Palliative Gamble: my brother has been initiated on a Somatuline (Lanreotide) injection, suggesting treatment for a rare subtype (Neuroendocrine). Doctors gave a blunt prognosis: 50% chance of stabilizing the disease. Current Supportive Care:

  • Medications: Somatuline, Diuretics (for fluid excretion), Albumin infusions (to stabilize circulation), and IV Nutrition/Sugars (due to his inability to eat, keep throwingup). All I know is sugar is terrible in his case. I don't know why they insist on giving him that.

The Questions (Looking for Miracles and Expert Experience):

A- after the latest biopsy results doctors were relieve it because they said that's NETs have a much better prognosis. They gave my brother the injection of Somatuline with hope of recovery. However, doctors said that there was not an immediate recovery after the first injection, and also the state of the liver had degraded. Now, doctors are thinking that perhaps he has a combination of NET and also a HCC. They said that they are looking into chemotherapy because it's the only option. They want to see if it's really HCC. They said that the states of my brother's liver is too fragile for standard chemotherapy so they are looking into giving them a diluted dose they said that it is still a risky maneuver and they need our approval.

Should we say yes? I have no idea what to say and I am genuinely terrified.

B- is there anyway to help improve the sate of his liver? By Nutrition? medication? Anything?

C- We are willing to try anything at this point. Even other non-traditionally recommended therapies. I am seeing testimonies online of people trying prolonged fasting, anti-parasitics, etc, with good outcomes. I need your help, please.

please reach out. I am desperate

Thank you for your help
