Hoping to get some clarification on immigration

To preface this I am a 29 year old male with diagnosed high functioning autism, and I'm currently unable to work. I live with my mother who is 56 and has a plethora of health problems from lymphedema to having had open heart surgery. We are both on social security disability from the United States. Is there any possible way for us to immigrate to the Czech Republic?
