People who climbed out of emerald, how?

We all know what is said about emerald. One game your teammates are good, following your calls all is nice and dandy, the next one your midlaner gets ganked once, you get spam pinged and you see in chat "ff15 jg diff". I feel like it's impossible to climb out of this elo unless you hard carry every game, which is impossible. So how did you do it?
