Moving to the Caribbean’s

So my fiancé keeps bringing up the idea of going to med school in the Caribbean’s.. we are residents of Tennessee currently. He would basically gain citizenship through college. As for me… I’m not sure I’m willing to part with him for 4 years, and I don’t know what I need to do to gain residency as I will not be attending a medical school there. Does anyone have any advice for me 😩
