Can't Contact Meta Support – Need Help Resolving an Issue. Check screenshot

I wanted to contact support with an issue I'm having, but I'm not able to contact them because there's an issue with the form. It says I have to fill the form, but I filled all the fields. Now I need to contact support to resolve this issue?
