Enlarged veins

I got this report from a neurosurgeon: “At the exit of the lumbosacral plexus through the suprapiriform foramen, there are bilaterally enlarged veins, which could explain your symptoms as they may contribute to compression of the PN. Otherwise, there is no sciatic split and no other relevant anbormalities visible on the scan”.

I’m offered surgery by transgluteal neurolysis, duringvwhich he decompresses the PN from the surronding veins by ligating them. Why not embalization??

Also its possible i have internal jugular veine, and that it is causing veine problems further down(but thats just me and my man AI discussing🥱). Any thoughts???
