What do I need to renew my driver's license if lost and surname change?

I have booked an appointment to renew my driver's license and they give the basics about what to bring with but I'm struggling to find any recent information about what to take with if:

a) You have lost your license and renewing early b) If you want to change/update your surname (already have a new ID)

If anyone has recently done either of these, some advice would be appreciated.

Thanks!
