Petition for Determination of Heirs

Location: Central Florida

A little bit of background: my mom passed away earlier this year and I was advised by a lawyer that I would likely need to completely a petition for determination of heirs. (My mom had a special needs trust and it lists the beneficiaries for any remaining assets as the heirs at law, but does not explicitly state my name or my deceased brother’s name, but we have no remaining living family.) I’ve tried to contact my local clerk of court to obtain this (central Florida) but get disconnected when I try to contact the probate office. I believe I was able to find the form but genuinely am not sure if it is the correct form, and also if it is, how to file it.
