SSS Sickness notification question

Hello po may question po ako regarding sa SSS notification. kapag po ba na confine po ako sa hospital for 3 days dahil na operahan and then after po nun home confinement na for 3 weeks. under na po ba ako dun sa 1 year or sa within 5 days notification pa din po?
