Different roles and specialization tracts for EM in different countries

As with all specialties, different countries have different roles and different systems for specializing. However, EM seems to have a wider variety than most. For example, do different specialists in your ED come and see 'their own' patients, or is the EM doctor fully in charge of the department? And is the EM specialty fully recognized, or is it only a additional course you can take after another specialty? Etc etc.

Dutch resident here. In our system every hospital seems to have a different organizational structure, but the training is more centralized and recently fully acknowledged as a specialty. Curious about your systems and your experiences with it.
