homework request before pre-rejection letter?

Are people typically notified of homework requirement before they get their 10-day notice pre-rejection letter? Or do they find out for the first time that the consulate wants something more/different in the 10-day notice letter? Curious about processes at different consulates. We had no notice that they changed their published document requirements after we had applied, until we got the 10-day pre-rejection notice.
